Healthcare Providers Portal
Welcome to the Department of Corrections (DOC) Healthcare Provider Portal. The Portal is intended for providers of onsite (i.e, inside prison facilities) and offsite (i.e., outside prison facilities) health care services to incarcerated individuals under the jurisdiction of the Department.
The Portal is a centralized resource center that includes information about 1) claims and billing, 2) fee schedules, 3) prison facility contacts to inquire about claims status, requests for medical treatment and rescheduling of appointments, and 4) updates regarding changes in policy and procedure that will affect incarcerated individual healthcare providers.
Continuity of Care Records Requests
A new health records request webpage is in development. In the meantime, if you are a healthcare provider and need to request records for continuity of care only, please use the DOC 13-507 Health Records Request Continuity of Care form. This form is for healthcare providers only. All other requests must be submitted through a Public Records request.

The DOC Health Plan defines which health care services are available to incarcerated individuals, as well as services that are limited or not available. The DOC Pharmaceutical Management & Formulary Manual governs the prescribing of medication to incarcerated individual. Neither the OHP or the Formulary are a contract or a guarantee of reimbursement for services provided to incarcerated individual.
